You can request a patient appointment using the form below.  During Office Hours we prefer that you telephone us, it is the way we can best be responsive to you.  If you reach our voice mail greeting, please leave us a message and we will call you back promptly. Call us at (616) 452-5721.

If it is presently outside of Office Hours, or it is more convenient for you to use the Appointment Request Form (below) rather than wait until the next business day, please feel free to do so. Online appointment requests will be contacted before the end of the next business day (consult our business hours below).

Beside Requesting an appointment, there are online communication options on our Contact Us page for sending us a message, asking questions, and letting us know you have interest in becoming a new patient.

New Patients

If you are a new patient, we will be asking you to complete a New Patient Information Form which will be needed for your first New Patient appointment.  You can get a jump on completing the New Patient Information in advance of your appointment, it will help expedite your first visit.
